The Opportunity
Care Compass is a new, transformative program. This integrated, regional, team-based model is designed to manage and coordinate care for Mass General Brigham’s high risk primary care patients. The Complex Care Manager RN serves as a core member of the care team. The RN coordinates and supports healthcare within the facility, and coordinates referrals for services outside the clinic.
