The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities:
Provides individualized assistance, support and comprehensive coordination of care for patients and their families/caregivers, to eliminate barriers to timely care, facilitate flow through system, facilitate interactions with care team, increase patient and provider satisfaction, maintain point of contact with providers and serve as first point of contact for patients and families.

Implements, coordinates, manages and evaluates a comprehensive urologic outpatient practice. This includes, but is not limited to, overseeing outpatient care plans, coordinating outpatient clinics, triaging patient calls and communicating with referring providers to ensure a safe and effective transition from referral through the postoperative period. This team member will work collaboratively with members of the division to ensure the delivery of safe quality driven care. This team member will also oversee the divisional performance improvement/quality assurance (PI/QA) program as it relates to urology specialty specific patient populations. Working together with the providers, nurse manager, and PI/QA coordinator, this team member will support the divisional efforts to ensure provision of safe and efficient care for this target patient population. This includes identification of initiatives, collecting, analyzing, tracking and trending of data to ensure compliance with divisional, departmental, and external quality initiatives. Registered Nurse (RN), BSN required, 3-5 years for nursing experience required, prior experience with urologic patients preferred.
