The Home Care Registered Nurse provides age-specific care for patients using the nursing process, collaborating with healthcare teams to implement and coordinate care plans. This role is ideal for nurses who are skilled in patient assessment and care management.

What You'll Do
ā¢Conduct patient interviews and explain policies and procedures.
ā¢Assess patients' health status across various systems.
ā¢Communicate and document patients' health status and care plans.
ā¢Identify patient outcomes and develop individualized goals.
ā¢Implement and reassess plans of care as needed.
ā¢Administer prescribed treatments and medications accurately.
ā¢Provide emotional support to patients and their families.
ā¢Participate in patient and family education and discharge planning.
Requirements
Education:
ā¢Graduate from an accredited School of Nursing required.
ā¢Bachelorās Degree in Nursing preferred.
Experience:
ā¢Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date.
Licenses & Certifications:
ā¢Current License to practice as a Registered Professional Nurse in New York State required.
ā¢Valid Driver's License in New York State may be required depending on area of assignment.
