The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.

Responsibilities:

GENERAL PURPOSE:

Accountable for provision of competent direct patient care based on established standards. Contributes to unit activities and supports enhancement of the care delivery system at the unit level. Supports peers in the delivery of patient care services and assumes an expanded role which may include charge nurse, preceptor, and committee member responsibilities.

FT/PT positions available, D/E/N, 12 hour shifts, holidays and weekends per program policy.

Patients enrolled in Hospital at Home receive inpatient hospital level care in the comfort of their own homes, rather than in a traditional hospital unit. Joining the Hospital at Home team is a rare opportunity to be part of opening and expanding a groundbreaking care model.  This program is newly launched and is looking for experienced, forward-thinking and creative RNs to pioneer it. Hospital at Home nurses will be trained to fill a variety of roles including charge RN, field RN and triage RN. Responsibilities include performing remote clinical triage, remote & in person patient assessment and administration of therapeutic interventions in patients' homes. H@H RNs will have the opportunity to develop a diversified set of skills. Individuals must possess strong acute care nursing skills, effective communication and have the ability to adapt to rapid changes. H@H offers opportunities for professional growth while working with multiple disciplines in a team-focused environment. The ideal candidate has enthusiasm for learning, patient-centered care and working in a team. Minimum 2 years of acute care, ICU and/or ED experience required. 5+ years of broad clinical expertise is preferred. This position requires a valid NYS driver’s license and reliable transportation.
