What You'll Do
ā¢Collaborate with a multidisciplinary care team to develop goals and plan interventions.
ā¢Provide care and disease management coordination.
ā¢Identify patients with ongoing coordination needs and conduct targeted outreach.
ā¢Conduct comprehensive clinical assessments addressing various patient needs.
ā¢Educate and coach patients and families on care coordination.
ā¢Serve as a liaison and advocate for patients and families.
ā¢Manage transitions of care across settings, ensuring optimal communication.
ā¢Identify barriers and connect patients to community resources.
Requirements
Education:
ā¢Graduate from an accredited school of Professional Nursing
Experience:
ā¢3-5 years of nursing experience
Licenses & Certifications:
ā¢Current state licensure as a Registered Nurse (RN)
ā¢Basic Life Support (BLS) Certification through AHA or American Red Cross
