Overview: We’re seeking dedicated and caring Case Coordinators to be part of a team with a patient-centered approach, in providing the most innovative care. This specialized role will assess transition needs of patients with a focus on stroke, traumatic brain, spinal cord injuries, along with other eligible diagnoses. The Case Coordinator will facilitate appropriate services across the care continuum, with the goal of returning the patient to their home environment. Our new unit features a collaborative extra professional approach to patient care. This is an excellent opportunity to work in a specialized inpatient rehab setting, while still having access to various providers within an acute care hospital.
