Join NYU Langone Health as a Clinical Coordinator (FGP) in the Center for Women's Health, where you will manage clinical triaging and coordinate services for fibroid patients, ensuring seamless care and efficient operations.

What You'll Do
•Conduct thorough assessments of patients' medical conditions and treatment needs related to fibroids.
•Screen patients seeking initial fibroid consult and gather relevant information.
•Pend orders for imaging and assist patients in scheduling required imaging.
•Collaborate with the Fibroid Center care team for effective communication and coordination.
•Serve as the primary point of contact for patients and their families throughout their treatment journey.
•Assist in obtaining timely medical clearance from PCP/specialists for surgical procedures.
•Perform weekly post-op calls to assess patient clinical status and manage expectations.
•Coordinate home care services as needed.
•Monitor patient outcomes, treatment compliance, and satisfaction levels.
•Maintain accurate and up-to-date patient records using the electronic health record system.
Requirements
Education:
•Bachelor of Science in Nursing (BSN) degree preferred
Experience:
•Proven experience as a Care Coordinator or in a similar care management role preferred
Licenses & Certifications:
•Valid and active Registered Nurse (RN) license in the state of practice
