What You'll Do
•Collect patient health data by involving the patient, family, and other healthcare providers.
•Perform initial assessments based on the patient's presentation.
•Record relevant data on the nursing component of the Interdisciplinary Admission Assessment.
•Demonstrate awareness of the special needs of each age group or culture served.
•Perform ongoing assessments as established by department policy.
•Incorporate discharge planning into the admission process.
•Analyze assessment data to identify problems and expected outcomes.
•Communicate expected outcomes to the patient, family, and other healthcare providers.
Requirements
Education:
•Professional knowledge of nursing theory and practice acquired through completion of a two-year training at an accredited School of Nursing.
Experience:
•1 year of nursing experience in dialysis care preferred.
Licenses & Certifications:
•Current New York State license as a Registered Professional Nurse.
•BLS certification required.
•ACLS preferred.
