About This Role

The Case Manager RN is responsible for coordinating and ensuring that the interdisciplinary plan of care and discharge plan align with the patient's clinical course and continuing care needs. This role is ideal for those who excel in collaboration and problem-solving within a healthcare setting.

What You'll Do

•Assess patient needs with the interdisciplinary team to develop a comprehensive management plan.
•Conduct performance management monitoring and participate in problem identification.
•Facilitate physician discussions with medical directors regarding third-party payer denials.
•Document clinical reviews and communicate with responsible medical and nursing staff.
•Coordinate post-discharge appointments to ensure continuity of care.
•Identify barriers to care and collaborate with operational leaders for improvement.
•Monitor patient progress toward goals and provide informal education to hospital personnel.
•Collaborate daily with the interdisciplinary team for assigned patients.

Requirements

Education:

•Bachelor's Degree required; combination of experience and education may be considered.

Experience:

•At least five years of relevant clinical experience with demonstrated leadership skills.
•Experience in Quality Improvement, Utilization Management, or Case Management preferred.

Licenses & Certifications:

•Current NYS Registered Nurse License required.
