About This Role
Join our team as a Field Nurse in Home Care for the Brooklyn territory, providing safe and quality nursing care using evidence-based practices. This role is ideal for candidates with at least one year of home care experience.

What You'll Do
•Completes comprehensive nursing assessments.
•Develops individualized care plans in collaboration with patients and healthcare teams.
•Coordinates care with other healthcare professionals.
•Delivers direct nursing care and ongoing patient education.
•Identifies barriers to effective patient education and engagement.
•Maintains accurate and timely documentation.
•Assists in achieving corporate goals and improving patient care outcomes.
Requirements
Education:
•Bachelor's degree in Nursing required for employees hired after 9/1/2020.
Experience:
•1 year of home care experience highly preferred.
Licenses & Certifications:
•Current and valid New York State Registered Professional Nurse license.
•Current and valid BLS certification.
