What You'll Do
•Complete comprehensive nursing assessments considering various patient factors.
•Deliver direct nursing care and ongoing assessment to patients and caregivers.
•Coordinate care by collaborating with the healthcare team.
•Maintain accurate documentation reflecting patient care and assessments.
•Identify barriers to patient education and employ strategies for engagement.
•Assist agency leadership in achieving quality patient care outcomes.
•Perform other duties as assigned.
Requirements
Education:
•Bachelor's degree in Nursing required for employees hired after 9/1/2020
Experience:
•1 year of home care experience highly preferred
Licenses & Certifications:
•Current and valid New York State Registered Professional Nurse license
•Current and valid BLS certification
