About This Role
Join NYU Langone Health as a Field Nurse in Home Care, where you will provide safe, quality nursing care and coordinate with an interdisciplinary team to promote optimal patient outcomes. This role is ideal for experienced RNs with a passion for home health nursing.

•💰 Pay: $110,966.74 - $160,466.74 annually

•⏰ Schedule: Per Visit
What You'll Do
•Complete comprehensive nursing assessments considering various patient factors.
•Derive diagnoses and collaborate with patients and families to develop care plans.
•Coordinate care with healthcare team members to optimize patient outcomes.
•Deliver direct nursing care and education to patients and caregivers.
•Identify barriers to patient education and employ strategies for engagement.
•Maintain accurate documentation according to regulatory requirements.
•Assist leadership in achieving corporate goals and improving patient care quality.
Requirements
Education:
•Bachelor's Degree in Nursing
Experience:
•At least one year of home care/home health experience
Licenses & Certifications:
•Current Registered Professional Nurse in New York State
•Basic Life Support Certification through the American Heart Association
