What You'll Do
•Coordinate services for post-acute care needs of patients.
•Ensure timely transitions to appropriate care settings.
•Collaborate with interdisciplinary patient care staff.
•Participate in quality improvement and evaluation processes.
•Maximize provider reimbursement through accurate insurance reviews.
•Perform utilization reviews and assess discharge needs.
•Develop and implement discharge plans with patients and care teams.
•Track risk and quality issues for improvement opportunities.
Requirements
Education:
•Bachelor's degree in Nursing required for employees hired after 9/1/2020
Experience:
•3 - 5 years of clinical acute care experience required
Licenses & Certifications:
•Current and valid New York State Registered Nurse License
•BLS certification (Issued by the American Heart Association-AHA)
