About This Role
We have an exciting opportunity to join our team as a Care Manager (RN) - Full-Time, Days. In this role, the successful candidate coordinates and procures services necessary to meet the post-acute care needs of patients, ensuring a smooth transition to the appropriate care setting.

What You'll Do
•Coordinate services for post-acute care needs of patients.
•Ensure timely transitions to appropriate care settings.
•Collaborate with interdisciplinary patient care staff.
•Participate in quality improvement and evaluation processes.
•Maximize provider reimbursement through accurate insurance reviews.
•Perform utilization reviews and assess discharge needs.
•Develop and implement discharge plans with care teams.
•Document all necessary information per departmental policies.
Requirements
Education:
•Bachelor's degree in Nursing required for employees hired after 9/1/2020
Experience:
•3 - 5 years in clinical acute care
Licenses & Certifications:
•Current and valid New York State Registered Nurse License
•BLS certification (Issued by the American Heart Association-AHA)
