About This Role
We have an exciting opportunity to join our team as a Case Manager RN - Part-Time, Days. This role is ideal for a candidate with clinical acute care experience who will coordinate and procure services necessary to meet the post-acute care needs of patients.

•💰 Pay: $71.87 - $88.00/hour
•📍 Location: Mineola, NY
•⏰ Schedule: Part-Time, Days
What You'll Do
•Coordinate services for post-acute care needs of patients.
•Ensure timely transition to appropriate post-acute care settings.
•Participate in quality improvement and evaluation processes.
•Perform utilization reviews and collaborate with interdisciplinary teams.
•Develop and implement discharge plans in collaboration with patients and care teams.
•Complete necessary documentation and track avoidable days for improvement.
•Provide patients with required notices and explanations of billing status.
•Facilitate communication among healthcare team members during patient care conferences.
Requirements
Education:
•Bachelor's degree in Nursing required for employees hired after 9/1/2020
Experience:
•3 years of clinical acute care experience required
Licenses & Certifications:
•Current and valid New York State Registered Nurse License
•BLS certification (Issued by the American Heart Association-AHA)
