What You'll Do
•Perform comprehensive nursing assessments.
•Develop individualized care plans in collaboration with patients and healthcare teams.
•Deliver direct nursing care and ongoing patient education.
•Maintain accurate and timely documentation of patient care.
•Assist in achieving corporate goals and improving patient care outcomes.
Requirements
Education:
•Bachelor's degree in Nursing required
Experience:
•1-2 years in acute care facility or home health, hospice, and long-term home health care program
Licenses & Certifications:
•Current Registered Professional Nurse in New York
•Basic Life Support Certification through the American Heart Association
