What You'll Do
•Perform comprehensive nursing assessments.
•Develop individualized care plans based on assessment data.
•Coordinate care with healthcare team members.
•Deliver direct nursing care and patient education.
•Identify barriers to patient understanding and engagement.
•Maintain accurate and timely documentation.
•Assist in achieving corporate goals and quality improvement initiatives.
Requirements
Education:
•Bachelor's degree in Nursing required
Experience:
•1-2 years in acute care facility or home health, hospice, and long-term home health care program
Licenses & Certifications:
•Current Registered Professional Nurse in New York
•Basic Life Support Certification through the American Heart Association
