About This Role
We have an exciting opportunity to join our team as a Field Nurse in the Home Care Division, focusing on Nassau/Suffolk. This role involves providing safe, quality nursing care and coordinating with an interdisciplinary team to promote optimal patient outcomes.

What You'll Do
•Perform comprehensive nursing assessments considering various patient factors.
•Develop individualized care plans in collaboration with patients and healthcare teams.
•Deliver direct nursing care and ongoing education to patients and caregivers.
•Maintain accurate documentation reflecting patient care and interventions.
•Assist leadership in achieving quality care goals and participate in improvement initiatives.
Requirements
Education:
•Bachelor's degree in Nursing required
Experience:
•1-2 years in an acute care facility or home health/hospice
Licenses & Certifications:
•Current Registered Professional Nurse in New York
•Basic Life Support Certification through the American Heart Association
