What You'll Do
•Completes comprehensive nursing assessments considering various patient factors.
•Derives diagnoses and collaborates to develop individualized care plans.
•Coordinates care with healthcare team members to optimize patient outcomes.
•Delivers direct nursing care and ongoing education to patients and caregivers.
•Identifies barriers to patient understanding and employs engagement strategies.
•Maintains accurate documentation according to regulatory requirements.
•Assists leadership in achieving corporate goals and improving patient care quality.
Requirements
Education:
•Bachelor's degree in Nursing required
Experience:
•1-2 years in acute care facility or home health, hospice, and long-term care
Licenses & Certifications:
•Current Registered Nurse License in New York
•Basic Life Support Certification through the American Heart Association
