About This Role
Join our team as a Per Diem Home Care Intake Nurse - Days, where you will complete comprehensive assessments of patients referred for home care services and ensure client eligibility for requested services.

•⏰ Schedule: Per Diem
What You'll Do
•Complete comprehensive assessments of patients referred for home care services.
•Plan to admit patients to the Agency and ensure eligibility for services.
•Obtain accurate information from healthcare records and caregivers.
•Collaborate with a multidisciplinary team to address transition issues.
•Identify potential safety risks and barriers to optimal functioning.
•Provide timely documentation reflecting intake coordination.
•Assist leadership in achieving corporate goals and improving patient care.
•Demonstrate caring behaviors towards patients and team members.
Requirements
Education:
•Diploma or Associate's Degree in Nursing
Experience:
•1-2 years in Acute Care Facility or Home Health, Hospice, and/or Long Term Home Health Program
Licenses & Certifications:
•Current and Valid New York State Registered Professional Nurse license
